Please call our office to schedule an appointment.

A written referral is required to take x-rays. Bring the completed form to your appointment.

Payment is due at the time of your appointment.  WE DO NOT BILL.  You will be quoted a fee when you schedule an appointment.

We will provide you with a receipt to submit for reimbursement from your insurance.

X-rays will be sent UPS courier to your doctor. All records will remain with your doctor.

You will need to remove jewelry and piercings around the head area for most procedures.

Notify technician if you are pregnant or suspect you are pregnant. We routinely do not x-ray pregnant patients during first trimester.

Please notify us in advance for any disability requiring special assistance.

24-hour notice of cancellation is appreciated. If more than 15 minutes late patient may be asked to reschedule
