Please call our office to schedule an appointment.
A doctor referral is required to take x-rays. Bring the completed form to your appointment. Click here to download the referral slip.
Payment is due at the time of your appointment. WE DO NOT BILL. You will be quoted a fee when you schedule an appointment.
We will provide you with a receipt to submit for reimbursement from your insurance.
All X-rays and records will be forwarded to referring doctor.
You will need to remove jewelry and piercings around the head area for most procedures.
Please notify the technician if you are pregnant or suspect you are pregnant. We do not x-ray pregnant patients during the first trimester.
Please notify us in advance for any disability requiring special assistance.
24-hour notice of cancellation is appreciated. If the patient arrives more than 15 minutes late they may be asked to reschedule.
